Healthy Pulse Care . ("Healthy Pulse Care," "we," "us," or "our") is committed to safeguarding your personal and health information. This Privacy Policy explains how we collect, use, disclose, and protect your information when you use our healthcare platform, website, and mobile applications (collectively, the "Services"). By using our Services, you consent to the practices described herein.
We collect personal details you provide directly: full name, email address, phone number, mailing address, date of birth, gender, Social Security Number (when required for insurance), and emergency contact details during account creation, appointment booking, and insurance enrollment.
We collect Protected Health Information (PHI) including medical history, current symptoms, diagnoses, prescriptions, lab results, imaging reports, treatment plans, allergy information, and clinical notes shared during consultations. All PHI is handled in strict compliance with HIPAA regulations.
We collect payment card details, bank account information, insurance policy and group numbers, and billing addresses for processing payments and insurance claims. Card data is processed through PCI DSS Level 1 compliant payment processors and never stored on our servers.
We automatically gather IP address, browser type and version, operating system, device identifiers, pages visited, time on page, click patterns, referring URLs, screen resolution, and interaction data with telehealth tools, chat features, and booking forms.
We use essential cookies for authentication and session management, functional cookies for preference storage, and analytics cookies for usage pattern analysis. We do not sell cookie data to advertisers. You can manage preferences through browser settings or our consent management tool.
Your information is used to: deliver and manage healthcare services; schedule and track appointments; facilitate telehealth consultations; process payments and insurance claims; send appointment reminders, test results, and follow-up instructions; personalize health recommendations; improve platform functionality; comply with legal obligations; detect fraud and security threats.
We use your contact information to send appointment confirmations, reminders via email and SMS, pre-visit preparation instructions, post-visit summaries, prescription notifications, and follow-up care recommendations. These are essential transactional communications.
When you use insurance services, we share necessary information with carriers, clearinghouses, and billing partners to verify eligibility, submit claims, obtain pre-authorizations, and process reimbursements under HIPAA's Treatment, Payment, and Healthcare Operations provisions.
With your explicit consent, we send promotional emails about new services, wellness programs, health campaigns, partnerships, and offers. You may opt out anytime via the unsubscribe link, our Unsubscribe page, or account preferences — without affecting transactional emails.
We share data with third-party providers performing functions on our behalf: cloud hosting (HIPAA-eligible AWS), payment processing (Stripe, PCI DSS Level 1), telehealth infrastructure (Zoom Healthcare), analytics (anonymized Google Analytics), and email delivery (SendGrid). All are bound by data processing agreements.
Your PHI is shared with licensed providers directly involved in your care, each bound by HIPAA, confidentiality obligations, and our Business Associate Agreements. Providers access only the minimum necessary information for your treatment.
We may disclose information when required by law: court orders, subpoenas, government requests; suspected child abuse reporting; public health reporting; health oversight agency audits; law enforcement requests involving imminent harm; and workers' compensation proceedings.
In a merger, acquisition, reorganization, bankruptcy, or asset sale, your information may transfer. We will notify you via email before your data becomes subject to a different privacy policy.
All data at rest is encrypted using AES-256 (FIPS 197). Data in transit uses TLS 1.3. Database access requires encrypted connections. Backup data is encrypted with separate key management.
Our HIPAA-eligible cloud environment uses dedicated VPCs, encrypted storage volumes, IAM policies, security groups, WAF protection, intrusion detection systems, and automated vulnerability scanning. Infrastructure is SOC 2 Type II certified.
PHI access requires multi-factor authentication, minimum 12-character passwords, session timeout after 15 minutes of inactivity, IP allowlisting for admin access, and role-based access controls. All access is logged in an immutable audit trail.
All employees undergo background checks, sign confidentiality agreements, complete HIPAA training upon hire and annually, and access data only per their role. Vendor personnel are subject to equivalent requirements through Business Associate Agreements.
We conduct annual penetration testing by independent security firms, quarterly vulnerability assessments, continuous security monitoring, and maintain an incident response plan tested semi-annually.
Active accounts: data retained while active. Health records: per state medical record retention laws (typically 7–10 years). Payment records: 7 years for tax compliance. Marketing data: until unsubscribe or deletion request. Inactive accounts: anonymized after 3 years of inactivity.
You may request a copy of all personal and health information we hold. Requests are processed within 30 days per HIPAA. Data is provided in readable electronic format through your patient portal or upon written request to our Privacy Officer.
You may request correction of inaccurate personal or health information. Health record corrections are coordinated with your treating provider. Submit requests through your portal or by contacting privacy@healthypulsecare.com.
You may request deletion subject to legal retention requirements. HIPAA-covered health records cannot be deleted during mandatory retention but will be restricted from further use. Non-essential data is permanently erased within 45 days of request.
You may request your data in a structured, machine-readable format (JSON/CSV). Health records can be exported as a Continuity of Care Document (CCD) compatible with any EHR system for seamless provider transfer.
You may request restriction of PHI disclosures for payment or operations when you've paid out of pocket in full. We accommodate reasonable requests as required by HIPAA.
Opt out of promotional emails, SMS, and push notifications anytime without affecting healthcare access or transactional communications. Use unsubscribe links, account preferences, or contact us. Processing within 10 business days.
We provide a separate HIPAA NPP detailing your rights under HIPAA, PHI use and disclosure practices, and our legal duties. Available in your patient portal and provided at first service.
California residents have additional CCPA/CPRA rights (know, delete, correct, opt out). Residents of Virginia, Colorado, Connecticut, and other states with privacy laws have analogous rights under applicable statutes.
Our Services are not for children under 13. We do not knowingly collect data from children under 13. If discovered, it is deleted immediately. Minors 13–17 require parental/guardian consent for account creation.
Data is stored primarily in the US. Any international transfers use Standard Contractual Clauses, adequacy determinations, and supplementary measures per applicable law.
In a PHI breach, we notify affected individuals without unreasonable delay and within 60 days per HIPAA. Notifications include breach description, data types involved, protective steps, and mitigation measures via email or first-class mail.
We may update this policy periodically. Material changes are communicated via email 30 days before taking effect. Continued use after changes constitutes acceptance.

These Terms and Conditions ("Terms") govern your access to and use of Healthy Pulse Care website, mobile applications, and healthcare services. By using our Services, you agree to be bound by these Terms. If you disagree, discontinue use immediately.
By creating an account, booking appointments, purchasing insurance, or using any Service, you confirm you are at least 18 (or have parental consent), legally competent, and accept these Terms as a binding agreement between you and Healthy Pulse Care.
Healthy Pulse Care provides a digital healthcare platform connecting patients with licensed providers for primary care, dermatology, dental care, haircare, weight loss management, fitness coaching, health insurance brokerage, and life insurance brokerage. Availability and pricing vary by state and provider.
You must be a US resident or territorial resident, at least 18 (or 13 with parental consent), not prohibited from receiving healthcare under applicable law, and capable of providing accurate information.
You must create an account for full access. You are responsible for credential confidentiality, all account activities, and prompt reporting of unauthorized access. We recommend two-factor authentication and complex passwords.
You represent that all information — personal details, medical history, insurance data, payment information — is accurate, complete, and current. False information may result in misdiagnosis, inappropriate treatment, claim denials, and account suspension.
Booking constitutes a request; confirmation is sent via email/SMS once finalized. Times are in the provider's time zone unless specified otherwise during booking.
Free cancellation/rescheduling 24+ hours before the appointment. Cancellations within 24 hours may incur a fee (up to $50, disclosed at booking). Repeated late cancellations may restrict booking.
If a provider cancels, we notify you immediately and offer the earliest alternative. No charge for provider cancellations. Pre-payments refunded in full within 5–7 business days if no alternative is available.
Missing an appointment without canceling incurs a fee (up to $75, shown at booking). Three or more no-shows within 90 days may result in account suspension.
Free rescheduling 24+ hours before. Within 24 hours is treated as late cancellation. Rescheduled times are subject to availability and not guaranteed at the original slot.
Prices are displayed before booking, vary by service type, provider, format (virtual/in-person), and location. All prices in USD, inclusive of applicable taxes unless stated. We may update pricing with 30 days' notice.
Visa, Mastercard, American Express, Discover, debit cards, HSA/FSA cards, Apple Pay, Google Pay. Payment processed at booking or service conclusion via PCI DSS Level 1 processors.
We bill your carrier directly with valid insurance information. You are responsible for co-pays, co-insurance, deductibles, and non-covered charges. If insurance denies the claim, you owe the full fee. We are not responsible for carrier decisions.
Payment due at service time. Overdue balances accrue 1.5%/month (18% annually). Failed payments incur a $25 processing fee. Accounts overdue 60+ days may have services suspended.
Refunds for provider cancellations, substantiated quality issues, and duplicate charges. Other refunds evaluated case-by-case. Approved refunds processed within 5–10 business days to the original payment method. Completed consultation fees are non-refundable.
Healthy Pulse Care is a licensed broker, not the insurer. We present plans, facilitate enrollment, and assist claims. Policy terms, coverage, exclusions, and premiums are set by carriers. Commissions are disclosed before purchase and do not affect your premium.
We act as a licensed broker for life insurance. We present options, assist applications and underwriting, and provide policy support. Coverage terms and premiums are set by issuing carriers. Commissions are disclosed pre-purchase.
Our total aggregate liability shall not exceed fees you paid in the preceding 12 months. We are not liable for indirect, incidental, special, consequential, or punitive damages including lost profits, data, or health outcomes.
Services are provided "as is" without warranties of merchantability, fitness for a particular purpose, non-infringement, or uninterrupted operation. We do not guarantee availability, error-free operation, or device compatibility.
Healthy Pulse Care connects patients with providers; we do not directly provide medical diagnosis, treatment, or prescriptions. All clinical services are delivered by independent, licensed practitioners solely responsible for their care. We do not guarantee health outcomes.
Our Services are not for medical emergencies. If experiencing a life-threatening emergency, call 911 or visit the nearest ER immediately. We are not liable for harm from failure to seek emergency care.
Telehealth requires: being physically in a state where the provider is licensed; stable internet and compatible device; privacy during consultation; acknowledgment that telehealth may not suit all conditions and in-person follow-up may be recommended.
All platform content, design, software, and code are our property or our licensors', protected by US copyright, trademark, and IP laws. No reproduction, distribution, or derivative works without written consent.
You agree not to: use Services unlawfully; provide false information; impersonate anyone; disrupt the platform; gain unauthorized access; harass providers or staff; record consultations without consent; or use automated scraping tools.
Your use is governed by our Privacy Policy, incorporated by reference. Using our Services constitutes consent to data collection and use as described, including PHI handling under HIPAA.
You agree to indemnify Healthy Pulse Care, its officers, directors, employees, agents, and service providers from claims, liabilities, damages, losses, costs, and expenses (including attorney fees) arising from your use, Terms violations, or third-party rights violations.
We are not liable for failures from circumstances beyond reasonable control: natural disasters, pandemics, government actions, wars, terrorism, labor disputes, power outages, internet disruptions, or third-party service failures.
Disputes first addressed internally via support@healthypulsecare.com. Unresolved disputes go to binding AAA arbitration in Boston, MA. You waive class-action lawsuits and class-wide arbitration.
Governed by USA law without conflict-of-law provisions. Non-arbitration actions filed in state/federal courts in USA. You consent to those courts' personal jurisdiction.
We may modify Terms anytime. Material changes communicated via email 30 days before effectiveness. Continued use after changes constitutes acceptance.
If any provision is unenforceable, it is limited to the minimum extent necessary. Remaining provisions stay in full force.
These Terms, with the Privacy Policy, HIPAA NPP, and service-specific agreements, constitute the entire agreement between you and Healthy Pulse Care regarding the Services, superseding all prior communications.

Unsubscribing removes your email from our promotional mailing list. Your account and access to all healthcare services remain fully active and unchanged.
Appointment confirmations and reminders, telehealth session links, prescription notifications, lab and test results, billing receipts, insurance claim updates, account security alerts, and policy change notifications continue as they are essential to service delivery.
Weekly health tips, seasonal campaigns, new service announcements, promotional offers, partner emails, survey requests, event invitations, and personalized recommendations will cease.
Requests are processed within 10 business days. A few promotional emails queued before processing may still arrive during this window.
A single confirmation email (transactional) will be sent once processed, confirming the effective date and what changed.
Re-subscribe anytime via account settings under "Communication Preferences" or the newsletter form. Re-subscription is immediate upon confirmation.
In account settings, choose: weekly digest only, monthly digest only, specific topics only, or new service announcements only — instead of full unsubscribe.
Unsubscribing from Healthy Pulse Care does not affect communications from insurance carriers or wellness partners. Unsubscribe from each partner separately.
Your data is retained per our Privacy Policy and HIPAA requirements (health records: 7–10 years). Your email is moved to a suppression list flagged "do not contact — promotional."
Full access to appointments, telehealth, prescriptions, insurance management, and all platform features continues unchanged.
Text message appointment reminders are transactional and continue. Opt out by replying STOP to any message or adjusting preferences in account settings.
App push notifications are managed in device Settings or app notification preferences, independent of email unsubscribe.
Provider messages, appointment updates, and test results displayed in the app are clinical communications unaffected by email preferences.
Clinical messages between you and providers are part of your medical record and are unaffected by marketing unsubscribe status.
Enrollment confirmations, premium notices, claim updates, and renewal reminders are transactional and continue for policy maintenance.
Breach notifications, suspicious activity alerts, password resets, and critical system updates cannot be unsubscribed — they protect your account.
Post-appointment satisfaction surveys cease. You can still provide feedback via your dashboard or support@healthypulsecare.com anytime.

Your access, correction, deletion, and portability rights under HIPAA, CCPA, and other laws remain regardless of subscription status.
We may send a verification email requiring confirmation to protect against unauthorized unsubscribes by third parties.
Rather than full unsubscribe, use our preference center for independent control: health tips, promotions, service updates, partner offers, surveys, seasonal campaigns — each toggleable.
Limited-time offers, discount codes, and flash sale notifications stop. Offers have expiration dates and may not be available upon re-subscription.
Flu prevention, summer wellness, back-to-school checkups, and awareness month emails stop once off the promotional list.
Past newsletters remain accessible via our website blog regardless of subscription status.
Your email is moved to a suppression list — not deleted (to prevent accidental re-addition) — flagged "do not contact — promotional" across all marketing systems.
We do not sell your email to third parties. Unsubscribing has no effect on data shared with healthcare or insurance partners for service delivery, which is governed by separate consents.
If you have multiple email addresses on file, unsubscribe only affects the specific email submitted. Unsubscribe each address individually or manage all in account settings.
We may periodically ask active subscribers to re-confirm their preferences. Non-response does not constitute consent and may result in removal from promotional lists.
